The MMSS program application consists of the following:
- Application Form
- Letter of Recommendation(s)
- Students Research Statement
- High School Transcript
- $100 Application Fee
There are various ways to apply to the MMSS program including:
1. Our Online Application (or)
2. Downloading the Application
If you choose our Online Application:
- Make sure you download the Letter of Recommendation (WORD), (PDF) and the Student Personal Statement (WORD), (PDF) (both can be mailed, faxed or e-mailed directly to the MMSS program at mmss@umich.edu.
- Complete the Application, making sure you hit the Submit button.
- Make arrangements to have a copy of your high school transcript sent to the MMSS program (a fax will suffice as well).
- Please send your $100 check (payable to the MMSS Program) with the application, making sure the student's name is on the check. This application fee will be applied towards the tuition and fees if you are admitted and returned to you if you are not admitted.
If you Download the Application (WORD) or (PDF):
- Mail the completed application to the MMSS program.
- Mail, fax or e-mail the Student Personal Statement and the Letter of Recommendation directly to the MMSS program.
- Make arrangements to have a copy of your high school transcript sent to the MMSS program.
- Please send your $100 check (payable to the MMSS Program) with the application, making sure the student's name is on the check. This application fee will be applied towards the tuition and fees if you are admitted and returned to you if you are not admitted.
